About the Role:

Personal Care Workers assist clients with daily living tasks such as bathing, dressing,
meal preparation, and basic household support.

Key Responsibilities:

• Provide personal care (bathing, dressing, hygiene).
• Assist with household tasks and errands.
• Document visit notes and report changes in client condition.
• Maintain client privacy and follow agency procedures.

Requirements:

• High school diploma or equivalent.
• Completion of agency training/competency testing.
• CPR certification (or willing to obtain).
• Must pass background check.

About the Role:

Homemakers assist clients with light housekeeping, companionship, and support with
errands or activities outside the home.

Key Responsibilities:

• Light housekeeping, laundry, and meal preparation.
• Assist client with shopping or errands.
• Provide companionship and emotional support.
• Document visit notes and report concerns to supervisor.

Requirements:

• High school diploma or equivalent.
• Training in homemaker/personal care services (provided if needed).
• Ability to communicate effectively.
• Must pass background check.

About the Role:

The RN provides clinical oversight and support to clients receiving home care
services. This role includes performing assessments, maintaining care plans, and ensuring
services are delivered according to regulatory and agency standards. The PRN RN is
scheduled on an as-needed basis, offering flexibility and autonomy.

Key Responsibilities:

• Perform initial and ongoing client assessments in the home environment.
• Develop, update, and review care plans to ensure appropriate service delivery.
• Provide clinical oversight to PCWs and Homemakers; assist with competency
  checks as needed.
• Communicate changes in client condition to leadership and/or family.
• Document all visits and assessments accurately and timely in accordance with
  agency policy.
• Maintain compliance with HIPAA and agency confidentiality standards.

Requirements:

• Current RN license in the state of Colorado.
• Minimum of 1 year nursing experience (home care or geriatrics preferred).
• Strong communication and clinical documentation skills.
• Ability to travel to client homes.
• Must pass a background check and meet agency credentialing requirements.
